Job summary

Brain Injury Services (BIS) seeks a full-time Brain Injury Case Manager to serve clients in the Northern Virginia region, including Prince William County and Fredericksburg. The role emphasizes complex case intervention, autonomy, and tight documentation within TCM/Medicaid workflows.

Responsibilities include intake, service authorization, ISP development, and ongoing client support with best-practice case management approaches. Travel to clients\' homes is required.

Qualifications

• Master's Degree required in a human service field such as Social Work, Rehab Counseling or Psychology.
• Brain Injury knowledge and related clinical understanding is essential.
• Experience with long-term case management and Medicaid/TCM processes is highly preferred.

Responsibilities

• Adhere to federal/local requirements and MCO regulations to serve and protect the agency.
• Complete intake evaluations and obtain medical records for eligibility.
• Process Medicaid TCM paperwork including authorizations, assessments, and billing.
• Develop and review Individualized Service Plans with client involvement and family support.
• Maintain monthly or quarterly contact with active clients and families; coordinate services and monitor progress.
• Travel to client homes and communities as needed; participate in discharge planning when possible.
