Job summary

Brain Injury Services seeks a full-time Brain Injury Case Manager to serve our clients in Northern Virginia. The role requires a professional level of intervention with complex cases and at least 2 years of case management experience, including Medicaid referrals, service authorizations, and collaboration with MCOs in TCM services.

The Case Manager will work under supervision to provide community-based services for adults and children with acquired brain injuries, traveling to clients' homes,

Qualifications

• Master's degree in a human services field (e.g., Social Work, Rehab Counseling, Psychology).
• Brain Injury Fundamental Training and Qualified Brain Injury Specialist training required on hire.

Responsibilities

• Adhere to professional standards, federal and local requirements, and MCO regulations.
• Obtain information and referrals for services, including Medicaid TCM referrals.
• Complete comprehensive intake evaluations and obtain medical records for eligibility.
• Process Medicaid TCM paperwork, including service authorizations, assessments, and billing.
• Develop a thorough client assessment and Individualized Service Plan within 45 days of opening a case.
• Maintain confidentiality and comply with privacy laws.
• Travel to clients' homes and communities and participate in discharge planning when possible.
• Maintain organized charts and ensure client consent signatures on reviews.
