Job summary

HCA Florida North Florida Hospital in Gainesville, FL seeks a Clinical Pharmacist BMT to provide comprehensive clinical pharmacy services for the Bone Marrow Transplant clinic, coordinating anti-neoplastic and immunomodulating therapies with the interdisciplinary team.

You will participate in multi-disciplinary rounds, develop protocols, educate learners, monitor immunosuppression, reconciliation, drug interactions, and transitions of care, while supporting quality projects and pharmacotherapy

Qualifications

• Pharmacist licensed or eligible for licensure.
• ACLS or BLS within 30 days of start date.
• Minimum 5 years experience preferred.

Responsibilities

• Provide comprehensive clinical pharmacy services for the BMT clinic.
• Coordinate anti-neoplastic and immunomodulating therapies with the care team.
• Educate patients on chemotherapy and supportive care regimens.

Job Summary and Qualifications

POSITION SUMMARY: The Bone Marrow Transplant (BMT) Clinical Pharmacy Specialist is responsible for the provision of comprehensive clinical pharmacy services for the BMT clinic. This individual serves as a resource for the BMT/Cellular Therapy, the interdisciplinary team, and other pharmacists. Duties will include the coordination, education, and medication management of anti‑neoplastic and immunomodulating medications, with oversight of organization‑wide collaboration on quality projects. Additional duties will include the coordination and application of anti‑neoplastic and immunomodulating medications, with management of organization‑wide collaboration on quality projects. Participation on multi‑disciplinary rounds including pharmacokinetic and therapeutic consultation will be performed.

1. Implement and monitor, with intradisciplinary healthcare team, patient medication therapy for achievement of desired therapeutic outcomes.
2. Help maintain accurate medication profiles for patients.
3. Provide patient education on chemotherapy and supportive care regimens.
4. Participate in protocol and order set development.
5. Participate in the education and training of healthcare students, residents, and providers.
6. Support the BMT/Cellular Therapy team in optimizing the care of their patients by providing meditation therapy optimization, patient education, and transitions of care services, including Immunosuppression monitoring, Medication reconciliation, Drug‑interaction screening, Evaluation of medication allergies, Medication adherence assessment, Patient and caregiver education, Medication access barriers, Chemotherapy appropriateness and dose adjustment evaluation.
7. Additional medication assessments, including Supportive care monitoring, Appropriateness of prophylaxis medications and immunizations, Antimicrobial stewardship, Oral chemotherapy management (if applicable), Steroid taper coordination, Education on non‑formulary medication use, Post‑transplant maintenance.
8. Opportunity to precept learners, including Student pharmacist, pharmacy residents, other healthcare professionals (if applicable).
9. Serve as a resource for the pharmacy team, nursing, and prescribers for questions/issues related to pharmacotherapy; analyze literature to support usage and contact prescribers as needed.
10. Support the BMT/Cellular Therapy team in optimizing the care of their patients by participating in intradisciplinary rounds, verification of chemotherapy and other medication orders, consultations, education, and transitions of care services.
11. Serve as a liaison between the Pharmacy and BMT/Cellular Therapy team/practices, collaborating to continuously improve medication use quality and safety.

• Advanced Cardiac Life Spt, or Basic Cardiac Life Support, or Bachelors Degree, or Doctorate Degree must be obtained within 30 days of employment start date
