Job summary

Advocate Health in Charlotte, NC, seeks an inpatient pharmacist to oversee pharmacotherapy, counsel patients, and ensure safe medication use across BMT, oncology, and pediatric satellite pharmacy settings. The role includes teaching learners and coordinating with the care team.

Requirements include a PharmD or ACPE-accredited degree, state licensure or reciprocity eligibility, and residency experience preferred.

Qualifications

• Pharmacy degree from ACPE-accredited school or NABP FPGEC certificate.
• Licensed in the state of practice or eligible for reciprocity.
• PGY2 Pharmacy Residency or equivalent in pediatrics or oncology; BCOP/BCPPs preferred.

Responsibilities

• Oversees pharmaceutical care, including literature review and therapy design.
• Counsels patients and caregivers on medication therapy, adverse effects and administration.
• Designs and monitors patient-specific regimens based on biomedical literature.
• Interprets medication orders and prescriptions accurately.
• Supports medication safety and quality improvement initiatives.
• Collaborates to manage formulary adherence and non-formulary acquisitions.
• Complies with policies, procedures, and regulatory requirements.
• Prepares, dispenses, and ensures proper storage/handling of medications.
• Serves as a preceptor and mentor for learners and staff.
