Merakey, a nonprofit provider of developmental, behavioral health and education services, seeks a Blended Case Manager to join our team in Monaca, PA.

You will serve as primary contact for up to 30 individuals in the community, develop personalized service plans, and coordinate with other agency providers while offering supportive counseling. On-call duties may be required.

Formación

• High school diploma or GED with >=12 sem. credit hours in social sciences.
• 2+ years’ experience in public or private human services; 1 year direct care.
• Valid driver's license and OMHSAS clearances.

Requirements

The ideal candidate will possess the following qualifications:

• High school diploma or GEDANDAT LEAST12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
• 2+ years’ experience in public or private human service, with 1 year being direct care required. Experiencemustbe with clients over the age of 18.
• Valid Driver's license required.
• All clearances required by OMHSAS regulations.

Preferred qualifications:

• Bachelor's degree in sociology, social welfare, psychology, gerontology, or other human services.
