Qualifications

  • 2+ years’ experience in public or private human services.
  • 1 year of direct care experience required.
  • Valid Driver's license and all clearances per OMHSAS regulations.

Position Details

Under the direction of the Blended Case Management Supervisor, you will:

  • Serve as the primary contact for up to 30 individuals in the community
  • Develop, coordinate, and implement personalized service plans
  • Provide case management, service planning, and coordination with other agency providers
  • Offer supportive counseling to help individuals achieve their goals
  • Work alongside county reentry grants to support individuals transitioning from jail back into the community
  • Participate in on-call responsibilities as needed

Qualifications

The ideal candidate will possess the following qualifications:

  • High school diplomaAND AT LEAST 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
  • 2+ years’ experience in public or private human service, with 1 year being direct care required.
  • Valid Driver's license required.
  • All clearances required by OMHSAS regulations.
