Job summary

Erie Family Health is seeking a Birthing Families Navigator to coordinate prenatal and post-partum care, outreach, and social services. You will assess risk, make referrals, conduct home visits as needed, and document in our health record systems.

Work is hybrid with Erie’s Division Street Health Center in Chicago. Strong collaboration with care teams, bilingual English/Spanish, and MS Office proficiency are required. Two years of related experience preferred.

Qualifications

• Bachelor’s degree in social work, child development, community health, or related field.
• Minimum two years' work experience in health, social or community services preferred.
• Intermediate proficiency in Microsoft Office; bilingual English/Spanish is required.

Responsibilities

• Provides coordination of primary care (prenatal, post-partum, pediatric care), outreach, follow-ups, risk assessments, referrals, and home visitation as needed; documents in funder data systems and EHR.
• Collaborates with fellow Birthing Families Navigators for continuous coverage and effective floor presence.
• Coordinates with client’s provider and care team and facilitates referrals and follow-up.
