Qualifications

Responsibilities

• Assess clients’ needs via a comprehensive assessment addressing social determinants of health.
• Develop case management plans and assist with care coordination.
• Facilitate trauma‑informed interventions and harm‑reduction approaches.
• Provide psychoeducation on mental health and substance misuse.
• Coordinate documentation and maintain HIPAA‑compliant records.
