North Range Behavioral Health in Greeley, CO is seeking a Bilingual Case Manager for the Access Team. You will conduct comprehensive assessments, connect clients to the appropriate level of care, and provide interim support to reduce barriers while awaiting ongoing services.

This full-time role requires a Bachelor's in a Human Services field, bilingual English/Spanish, a valid driver's license, and ability to complete background checks and TB testing.

Qualifications

• Bachelor's degree in a human services field from an accredited college or university.
• Bilingual in English and Spanish.
• Valid driver's license and an insurable driving record.
• Pre-employment screening including background check, fingerprinting, and TB testing.

Responsibilities

• Support clients in reaching treatment goals and interim interventions.
• Coordinate and monitor care with therapists and staff to prevent deterioration.
• Refer, coordinate, and assist clients in accessing services such as vocational training, medical care, social security, and housing.
• Build cooperative relationships with community organizations and agencies to deliver services.
• Enhance client engagement by teaching psychoeducational curricula and adapting techniques.
• Promote daily living skills, including financial management and self-care.
• Provide caregiver support to family members and reduce barriers to outcomes.
• Adapt to client needs with flexible service delivery including home visits and appointment accompaniment.
• Develop client coping skills and problem-solving strategies.
• Monitor progress and maintain records in electronic systems.
