Jewish Family Service of Colorado is seeking a Bilingual Housing Case Manager in Denver, Colorado, to provide housing-focused stabilization and case management services. Qualifications

• Three+ years of social case worker experience.
• Experience with adults with IDD preferred.
• Spanish bilingual required.
• Proficient in Microsoft Office.
• Knowledge of Denver metro resources preferred.

Responsibilities

• Provide housing-focused stabilization case management to help clients secure permanent housing.
• Develop, implement, monitor, and modify service plans with clients.
• Collaborate with other case managers, doctors, organizations, and families to resolve complex needs.
• Assist clients in navigating disability determinations and enrolling in Medicaid waivers.
• Document client contacts and progress in required CRMs.
