Job summary

Houston Area Women’s Center is seeking a Transitional Housing Case Manager to provide trauma-informed case management, crisis intervention, and individualized support for survivors of domestic and sexual violence in a 100% onsite role.

You will work Monday 12 p.m. - 9 p.m. and Tue–Fri 9 a.m. - 6 p.m., with local travel up to 25%. The position offers medical/dental/vision, life and disability insurance, generous PTO, 401(k) with 125% match, and agency-paid life insurance.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Counseling, Sociology, Criminal Justice, Public Health or related field.
  • Minimum 1 year experience in case management or DV/sexual violence support in a social service setting.
  • Knowledge of domestic/sexual violence issues and community resources preferred.
  • Familiarity with Texas family violence laws and justice system responses preferred.
  • Texas driver’s license and reliable transportation; travel to off-site locations required.

Responsibilities

  • Provide trauma-informed case management to survivors in the Transitional Housing program.
  • Develop individualized action plans and connect clients to resources for safety, stability, and housing.
  • Maintain accurate client documentation in compliance with policies and funder requirements.
  • Collaborate with staff to ensure a safe, supportive environment for clients and families.
  • Attend weekly Case Team Meetings and manage a caseload of 19–24 families.

As the Transitional Housing Case Manager you will provide empowering and supportive services to survivors of domestic and sexual violence residing in the Transitional Housing program. This role focuses on delivering trauma-informed case management, crisis intervention, and individualized support to help clients achieve safety, stability, and long-term independence. The Transitional Housing Case Manager will work collaboratively with clients to identify goals, connect them to resources, and support their transition to permanent housing. Every day we continue to embed diversity, inclusion, belonging, and equity in everything we do as we provide service to survivors of domestic and sexual violence.The Requirements We are Seeking:Bachelor’s degree in Social Work, Psychology, Counseling, Sociology, Criminal Justice, Public Health, or other social science.A minimum of 1 years’ experience working within case management, or working with survivors of domestic and sexual violence, or similar in a social service setting working with clients from diverse backgrounds.Knowledge of and sensitivity to issues of domestic and/or sexual violence, preferred.Knowledge of community resources for adults and children needing emergency services including legal, financial support, housing and/or basic needs, preferred.Working knowledge of Texas laws pertaining to family violence, as well as the justice system's response to domestic violence, preferred.Familiarity with legal services, resources, and procedures available to victims in each county where services are providedProven skill of crisis management is preferredProficiency in Microsoft Office (Word, Excel, Outlook, and Teams).Texas driver’s license with a good driving record; ability to travel to off-site locations, utilizing personal transportation. MVR required – Must be able to drive vanWhat you Deliver in this role:You will plan and implement safe, developmentally appropriate activities for the children in the program using the four developmental domains (cognitive, language, physical, and socio-emotional).You will provide positive guidance to children that include the following practices: redirecting, modeling, discussion, and positive reinforcement.You will maintain a safe and nurturing learning environment that promises a commitment to children and supports a variety of learning opportunities where children are free to discover and explore.You will communicate daily with parents about their children and give appropriate guidance, when necessary, regarding early childhood development.You will work within the Child and Adult Care Food Program guidelines to ensure that each child receives optimal nutrition.You will recognize, document, and take appropriate action in case of suspected abuse, illness and/or accident, reporting the incident.You will assist with play time, after-school, summer enrichment program, field trips and special events.You will ensure compliance with all grantors standards, Texas Department of Protective and Regulatory Services, Houston Area Women’s Center policies and procedures, and all other contract standards as required.Other duties may be assigned.What Skills and Assets you bring, and more:Empower, support, and monitor the progress of domestic and sexual violence survivors in the Transitional Housing program, ensuring services align with agency, funder, and state/federal guidelinesProvide ongoing trauma-informed case management to assist clients in identifying and achieving goals related to safety, stability, and securing permanent housing.Meet regularly with clients to assess needs, develop individualized action plans, address barriers, and connect them to community resources, employment opportunities, and housing services.Maintain accurate, timely, and detailed client documentation in compliance with agency policies, funding requirements, and state/federal guidelines.Collaborate with Transitional Housing staff and other departments to ensure a safe, supportive, and positive environment for clients and their families.Foster an environment of diversity, equity, and inclusion that respects the unique needs and experiences of all clients.Serve as a liaison between clients and community partners, providing referral information for legal, financial, employment, housing, and other supportive services to help clients successfully transition to long-term housing solutions.Attend weekly Case Team Meetings to share feedback, discuss client progress, and develop coordinated plans for addressing client needs.Manage a caseload of approximately 19-24 families, ensuring consistent and high-quality support throughout the clients’ stay in the program.Monitor and report any concerns of abuse and/or neglect to the appropriate authorities, including Child Protective Services (CPS) or Adult Protective Services (APS), in accordance with agency policies and legal requirements.Advocate for clients by assisting them in understanding their legal rights and accessing resources that promote self-sufficiency and independence.Support the overall success of the Transitional Housing program by performing additional duties or projects as assigned.When you work here at HAWC you make an impact not just in the lives of the survivors and the people you help, but also make an impact in our own lives and our community.
