Job summary

Ascend Healthcare is seeking a fully remote, bilingual licensed therapist or counselor (LPC, LCSW, LMFT) to provide counseling services to patients through telemedicine for partner Community Health Centers. Must obtain or hold active licenses in Arizona or Oregon in addition to your primary state license.

The role emphasizes evidence-based modalities, comprehensive documentation, crisis intervention, and collaboration with a multidisciplinary team to support recovery and stabilization.

Qualifications

• Active, unrestricted mental health license in state of primary residence and state of medical services being provided.
• Comfortable working in a remote environment.
• Bilingual English/Spanish (Required).

Responsibilities

• Provide patient care using evidence-based modalities (CBT, DBT, Solution-Focused Therapy, Psychoeducation, Trauma Therapy, Addiction Therapy, ACT).
• Deliver individual and group therapy with engagement in the treatment plan.
• Review and sign psychosocial assessments, diagnoses, and treatment plans, maintaining accurate records.
• Document all services and activities in accordance with regulatory and contractual requirements.
• Conduct crisis intervention as needed.
• Participate in multidisciplinary case planning and supervision.
• Provide case management to promote recovery and community reintegration.
• Perform additional duties as assigned.
