Job summary

Mom's Meals is seeking a Nutrition Educator to provide telephonic nutrition education to members with chronic conditions, helping them select, prepare and eat meals that fit their health goals. You will collaborate with members to explore nutrition issues and connect them with resources in their community.

The role requires a BS in Nutritional Science or Dietetics, RD licensure, Spanish fluency, and at least two years of counseling experience.

Qualifications

• Bachelor of Science in Nutritional Science or Dietetics.
• Registered Dietitian certification by the Commission on Dietetic Registration.
• Licensed or eligible for licensure in the state of residence.
• Minimum of 2 years' experience providing nutrition education or counseling.

Responsibilities

• Provide telephonic nutrition education sessions per member/patient; number of sessions varies by program.
• Document goals and care plans from education sessions in the platform.
• Schedule follow-up calls with each member/patient.
• Develop nutrition education handouts and tools as needed.
• Identify community resources for clients as needed.
• Contribute to social media content and articles for the company newsletter and website.
