Job summary

ICONMA is seeking a Case Management Coordinator for our client, a Retail Pharmacy in the Miami area, to coordinate Medicaid Long Term Care/Comprehensive Program enrollees' care.

The role requires 1 year of case management experience, bilingual Spanish/English, and proficiency in Microsoft Office. The candidate will develop and monitor care plans, educate members, and collaborate with providers to achieve optimal outcomes.

Qualifications

• One year case management experience required.
• Case Management Certificate preferred.
• Proficient in Microsoft Office including Excel.

Responsibilities

• Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
• Collaborates with care teams to inform the case management process and support members.
• Evaluates Members using care management tools and data.
• Coordinates and monitors care plan progress.
• Conducts multidisciplinary reviews to achieve optimal outcomes.
• Identifies and escalates quality of care issues through established channels.
