Job summary

Randstad USA is seeking a self-motivated Case Management Coordinator in the Miami area to manage and coordinate care for members on a Monday–Friday schedule. The role involves telephonic and face-to-face assessments, planning and implementing care to enhance overall wellness, with a heavy travel component within Miami-Dade County.

Responsibilities include coordinating with PCP and providers, handling authorizations, reviewing medications, and linking to community supports.

Qualifications

• Bilingual Spanish/English fluent speaking and writing.
• Ability to multitask, prioritize and adapt in a fast-paced environment.
• Effective verbal and written communication skills.
• Candidate will travel approximately 75% of the time within Miami-Dade County.

Responsibilities

• Telephonically and/or face-to-face assessing, planning, implementing, and coordinating all case management activities with members to evaluate medical needs and support overall wellness.
• Manage a caseload including supportive and medically complex members and develop proactive action plans.
• Determine appropriate services and supports: Prior Authorizations, coordination with PCP and skilled providers, medication review, community resources and supports.
