Job summary

COMHAR is seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team in the Philadelphia area. You will supervise Targeted Case Managers, guide service delivery, ensure program standards compliance, and coordinate care for adults in the community.

You will foster a collaborative environment and ensure high‑quality, client‑centered services with strong documentation and staff supervision responsibilities.

Qualifications

• : Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling, or education.
• : Knowledge of Blended Case Management Standards of Care.
• On-call responsibility and completion of BHTEN 10-week case management orientation; valid driver’s license required.
• Bilingual proficiency in English and Spanish, with strong reading and writing skills.
• Current Driver's License; satisfactory driving record; eligible to operate program vehicles.

Responsibilities

• Blended Case Management Supervisor is responsible for ensuring that the DBH Practice Guidelines are implemented to the extent possible within the general operations of the TCM duties.
• Provide daily on-site supervision to teams of Blended Case Managers, including team meetings to review proposed daily contact for each staff member and address issues with team members as needed.
• Provide weekly individual supervision to each assigned staff, with documentation and chart review using the updated Utilization Review sheet, including review of PGP coverage since the last CBH review.
• Monitor the 30-day contact board daily and ensure contacts are completed to prevent infractions.
• Ensure Pay 4 Performance criteria are met.
• Oversee timely, legible documentation that is signed, dated, uses the DAP format, and describes the nature of each contact.
• Monitor staff productivity using reports provided by the TCM director.
