Job summary

Crescentcare in New Orleans is seeking a Spanish Speaking Social Worker or Social Services Case Manager. The role focuses on integrating care for clients, ensuring access to medical and behavioral health services, and requires fluency in Spanish.

Qualifications

• Currently licensed social worker (RSW, CSW, LMSW, LCSW) or licensed nurse (RN or LPN) in Louisiana.
• One year post-degree experience in direct service to HIV target population.
• Availability for 40 hours of work per week.

Responsibilities

• Assist in the full implementation of the program.
• Document all interactions with clients in the electronic health record.
• Collaborate with the treatment and recovery team.

Skills

Fluency in Spanish
