Job summary

Catholic Social Services in Columbus, OH is seeking a Social Worker to provide case management to Spanish-speaking residents on the Westside of Franklin County. The role includes intake, goal setting, service planning, progress monitoring, advocacy, and referrals.

Experience supervising staff/interns and collaborating with partners to enhance services is required. Full-time, Mon–Fri, with competitive benefits and retirement plan.

Qualifications

• Provides intake assessment, goal setting, case plan development, progress monitoring, advocacy, and referrals.
• Meets with clients in their home and/or community as directed by the case plan.
• Inputs accurate and complete documentation for all contacts with clients into agency database.
• Supervises staff and interns including onboarding, case assignment, clinical, coaching, and administrative support.
• Sets individual and team performance goals with the Director.
• Builds partnerships with community partners to enhance resources for human and social services.
• Delivers comprehensive case management for Spanish-speaking clients including intake, goal setting, service plan development, progress monitoring, advocacy, referrals, and linkage.

Responsibilities

• Provide comprehensive case management to all clients including intake, goals, plans, progress monitoring, advocacy, and referrals.
• Meet clients in home/community per case plan directions.
• Document all client contacts accurately in the agency database.
• Supervise staff and interns: onboarding, case assignments, clinical coaching, and admin support.
