Job summary

Far-Northern-Regional-Center is seeking a Service Coordinator CU (Over 6) in Redding, CA. The role focuses on case management for individuals with developmental disabilities, including home visits, IPP development, and interagency coordination.

Qualifications

• Requires equivalent to a B.A. Degree in field related to Social Welfare
• Must demonstrate fluency in English and Spanish to qualify for bilingual compensation (ability to speak, read, and write)

Responsibilities

• Provides case management services to an assigned group of persons with developmental disabilities
• Develops and monitors Individual Program Plans
• Coordinates services with other agencies and service providers
• Conducts home visits with clients and families
• Consults with clinical and other professional staff
• Advocates on behalf of clients
• Completes reports and forms; Maintains case records
• Other duties as assigned
