Job summary

Far-Northern-Regional-Center in Chico, CA is seeking a Service Coordinator CU (Over 6) for the Children\'s Division. This full-time position handles case management for individuals with developmental disabilities, develops and monitors IP plans, and coordinates services with partner agencies.

Qualifications

• BA degree in Social Welfare or related field or 1 year of related experience.
• Fluency in English and Spanish to qualify for bilingual compensation.

Responsibilities

• Provide case management for an assigned group of persons with developmental disabilities.
• Develop and monitor Individual Program Plans.
• Coordinate services with other agencies and service providers.
• Conduct home visits with clients, families or other care providers.
• Advocate on behalf of clients.
• Complete reports and forms.
• Maintain case records.
