Job summary

Compass Connections seeks compassionate registered nurses to join the Heritage facility in San Antonio, TX. The role emphasizes care coordination, patient safety, documentation, and direct nursing care under federal and state guidelines.

Requirements include a graduate of an accredited nursing program, an active Texas RN license, BLS (PALS preferred), CPR, and the ability to speak English and Spanish. Work schedule is Monday–Friday, 8am–5pm.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Active RN license in Texas; BLS/PALS preferred; CPR required.
• Minimum 1 year in public health or healthcare with care coordination experience.

Responsibilities

• Meet federal/state guidelines and standards for health services.
• Document medical records accurately per PHI and regulatory rules.
• Assess, triage, monitor health conditions and administer vaccines/meds as ordered.
• Coordinate transportation and interpretive services for patient care assistants.
• Maintain confidentiality and comply with safety protocols.
