Job summary

Intermountain Health St Vincent Regional Hospital in Billings, MT seeks an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings. You will assess, plan, implement, and monitor patient-centered care plans with an emphasis on efficient, effective care and collaboration with providers, payers, and families.

Qualifications

• Current RN license in state of practice.
• BSN required within four years of hire; education agreement precedence.
• Inpatient hospital nursing experience with hospital terminology.
• Understanding of disease management and care delivery barriers.
