Job summary

Clinica Family Health & Wellness is seeking a Registered Nurse to support the Behavioral Health Team with nursing assessment, case management, and patient education. You will provide care at the top of your scope and collaborate with providers to achieve healthy outcomes.

The role requires a Colorado RN license and bilingual English/Spanish, with a supportive team and opportunities for professional growth in a mission-driven clinic.

Responsibilities

• Attend to the needs of the team including injections, medication administration and patient education.
• Conduct independent nurse visits such as wound care and hypertension visits.
• Follow up with abnormal lab results and treat per protocol.
• Co-visits with providers to evaluate and treat acute patient needs.
• Triage walk-in patients.
• Attend pre-shift huddles to discuss anticipated needs.

As our Registered Nurse, you will work with our Behavioral Health Team providing nursing assessment and case management. You will support complex care for patients, engage in client advocacy and contribute to the plan of care that helps clients to reach healthy outcomes.

What You Get to Do:

• Attend to the needs of the team including injections, medication administration and patient education.
• Work at the top of your scope doing independent Nurse Visits such as wound care, managing INRs, Hypertension visits, UTI’s, etc.
• Follow up with abnormal lab results and treating per protocol.
• Co-visits where you will evaluate patients, in collaboration with the provider, and treat patients with acute needs to help create access for patients.
• Triage any walk-in patients.
• Build teamwork by attending huddles before the start of shifts to review any anticipated nurse needs during that shift.

Compensation:

$34.70 - $43.95/hour. The posted rangerepresentsthe fullsalary rangefor this position. Starting pay is based on factors suchasdirectlyrelatedexperience, education, licensure and/or certification, and internal equity.
