Job summary

Angels of Care Pediatric Home Health is seeking a Clinical Care Manager - RN in San Antonio, TX to lead a team of clinicians and oversee client care. You will manage a caseload, coordinate with an interdisciplinary team, and train new hires to uphold high standards of service.

The role requires an RN, a nursing degree from an accredited program, and preferred home health or pediatric experience. Bilingual English/Spanish is a plus; travel is local as needed.

Responsibilities

• Supervise and support direct care staff to ensure policy and regulatory compliance.
• Manage a caseload of clients, develop personalized treatment plans and coordinate care with external providers.
• Conduct comprehensive assessments and supervisory visits, documenting care plans per agency standards.
• Lead orientation, training and competency verification for new hires.
• Act as liaison among staff, community and healthcare providers to ensure seamless communication.
• Provide in-field supervision and real-time guidance to clinicians; participate in QA and staff development initiatives.
• Support on-call operations with timely follow-up and accurate documentation.

Anticipated Compensation: $76,000 to $81,000

Schedule: Full-time, Monday–Friday, daytime business hours

Travel: Regular local travel required.

As a Clinical Care Manager - Registered Nurse (RN), you will lead a team of dedicated clinicians in delivering exceptional client care. You will manage a case load of clients, partner with an interdisciplinary team, train and onboard new hires, and manage client referrals.
