Qualifications

• Bilingual proficiency in Spanish and English is required.
• Minimum 1–3 years of residential support or case management experience.
• Experience conducting assessments, crisis intervention, and coordinating services for mental health and substance use needs.
• Education aligned with human services (HS diploma or higher) and relevant experience.

Responsibilities

• Partner with clients to develop individualized service plans in English and Spanish.
• Coordinate access to mental health, substance use, and housing resources.
• Provide community and home-based support using clinical judgment.
• Participate in case reviews to ensure service quality and stability.
• Assist clients with housing access, landlord relations, and financial assistance.
• Document in accordance with funder and agency standards.
• Promote family well-being through school, recreation, and healthcare links.
