Responsibilities

• Provide medical and personal care for the client based on the individualized Plan of Care prepared by the Nurse Supervisor available in the client’s home, ensuring client’s needs are met in relation to safety, well‑being and satisfaction.
• Orient new caregivers to cases and clients’ plan of care, ensuring clients’ clinical needs are met.
• Provide prompt communication of changes in patient medical status/emergencies to the Nurse Supervisor, ensuring effective case management.
• Provide prompt communication of changes in clients/caregivers schedules (e.g., tardiness, vacation, requested time‑off, change of status, reduction in hours, etc.) to Staffing Coordinators, ensuring effective case management.
• Drive/escort client to and from appointments/activities as requested, utilizing client vehicle, ensuring safe and timely transport.
• Call Staffing Coordinator to confirm weekly schedules, ensuring required coverage is met.
• Provide accurate and timely required documentation (e.g., decline to participate in all parts of Plan of Care), ensuring effective management of cases and DHR compliance.
• Other duties as assigned or needed.

Qualifications / Requirements
