Job Description

To provide the highest quality of care to Accord Services clients utilizing the Plan of Care provided to ensure client’s needs are met in relation to safety, well‑being and satisfaction.

Essential Job Responsibilities
  • Provide medical and personal care for the client based on an individualized Plan of Care prepared by the Nurse Supervisor available in the client’s home, resulting in satisfaction for all parties involved.
  • Orient new caregivers to cases and clients’ plan of care, ensuring client’s clinical needs are met.
  • Provide prompt communication of changes in client medical status or emergencies to the Nurse Supervisor, ensuring effective management of cases.
  • Provide prompt communication of changes in clients’ or caregivers’ schedules (e.g., tardiness, vacation, requested time‑off, change of status, reduction in hours) to Staffing Coordinators, ensuring effective management of cases.
  • Drive or escort client to and from appointments or activities as requested, utilizing client vehicle, ensuring safe and timely transport.
  • Call in to Staffing Coordinator to confirm weekly schedules, ensuring required coverage is met.
  • Provide accurate and timely required documentation (e.g., declining to participate in all parts of Plan of Care), ensuring effective management of cases and DHR compliance.
  • Other duties as assigned or needed.
Qualifications

LPN requirements:

  • High school diploma or GED.
  • CPR certification.
  • Current TB skin test or Chest X‑ray.
  • Certificate from an accredited nursing program.
  • Valid driver’s license (if assigned to a case with driving privileges).
  • Previous experience is preferred, but not required.
