Qualifications

• High school diploma or GED is required.

Responsibilities

• Provide medical and personal care for the client based on the individualized Plan of Care.
• Orient new caregivers to cases and clients’ plan of care.
• Provide prompt communication of changes in patient medical status to the Nurse Supervisor.
