Job summary

ADP, Inc. in Lakewood, WA is seeking a bilingual Primary Care Physician to join a full-risk, value-based care practice. This full-time role emphasizes relationship-based care, proactive health management, and personalized patient engagement for Commercial and Medicare patients.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Active and unrestricted license in the state of California.
• Current DEA and NPI registration.

Responsibilities

• Deliver comprehensive, longitudinal primary care to a panel of Commercial and Medicare age patients.
• Provide patient-centered appointments, allowing time for whole-person care, counseling, education, and shared decision-making.
• Conduct Annual Wellness Visits (AWVs), preventive screenings, and evidence-based chronic care visits in alignment with CMS and other health plan quality metrics.
• Develop and maintain personalized care plans based on patient goals, risk factors, and social determinants of health.
• Integrate behavioral health, social support services, and care management into care plans in collaboration with the interdisciplinary team.
• Coordinate with Care Management, Pharmacy, Behavioral Health, and Social Work teams for complex patient needs and transitions of care.
