Qualifications

• MSN, DNP, or NP certificate program completed.
• Active NP license and DEA registration.
• US work authorization and Spanish bilingual proficiency.
• Experience in value-based primary care is a plus.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments.
• Care coordination with other providers, testing facilities, and agencies.
• Population health leadership with the care team.
• Assist with phone triage and outreach.
• Educate patients on health conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
