This role supports provider and patient needs, educates on screenings gaps, schedules appointments, improves clinical workflows, and participates in community outreach and grant opportunities.

Responsibilities

• Contacts patients identified with unmet needs.
• Educates patients to close screening gaps.
• Encourages scheduling PCP visits to close gaps.
• Provides information to overcome concerns/barriers to care.
• Schedules appointments based on QI reports.
• Follows scheduling scripts when calling patients.
• Aids in optimization of clinical workflows.
• Outreach in the community for patient engagement.
• Identifies grant opportunities to support closing care gaps.
• Other duties as assigned.
