Qualifikationen

• Conduct monthly home visits for program members and their Care Providers, ensuring well-being and regulatory compliance.
• Complete all necessary documentation accurately and promptly to maintain thorough records.
• Maintain open lines of communication with assigned staff and coordinate with other service providers.
• Make referrals to external resources when additional support is needed.
• Monitor the home setting according to regulatory guidelines for safety and comfort.
