The Arc of Opportunity is hiring a full-time Person-Centered Plan Manager for Shared Living in Fitchburg, MA. You will support care providers, conduct monthly home visits, and ensure compliance with program regulations.

Qualifications

• Bachelor's degree or equivalent experience in social services or related field.
• Background checks and pre-employment screenings including TB test and CORI.
• Basic computer skills and documentation entry.
• Strong interpersonal and communication skills with program participants and staff.
• Valid driver's license and reliable vehicle meeting state requirements.

Responsibilities

• Conduct monthly home visits for program members and their Care Providers.
• Complete documentation accurately and promptly to maintain records.
• Coordinate with staff and other service providers to ensure comprehensive care.
• Make referrals to external resources when additional support is needed.
• Monitor the home setting for safety and regulatory compliance.
