Job summary

Vista Community Clinic (VCC) is seeking a Referral Coordinator to guide patients through dental evaluations and coordinate with other departments to ensure seamless care. The role includes collecting information, verifying eligibility, scheduling specialists and assisting with program applications.

Qualifications

• High school diploma or equivalent.
• Two years’ experience in medical, social service setting or related field.
• Bilingual English/Spanish.

Responsibilities

• Guide referrals for dental evaluations and coordinate with other departments.
• Complete paperwork and verify insurance and eligibility for referrals.
• Provide scheduling information and contact instructions for specialists.
• Data collection, data entry and program reporting support.
• Interact with patients and caregivers to resolve referral issues.
• Assist patients in applying for health programs (BCCTP).
• Support planning and implementing program strategies.
• Assist supervisor with data analysis and reporting.
