Job summary
Oak Street Health seeks a Nurse Practitioner to provide effective, value-based primary care to Medicare adults at our Jamaica, NY clinic. You will practice independently within a collaborative team, building lasting patient relationships and guiding care plans to keep patients happy and healthy.
The role requires an MSN/DNP or NP certificate, valid NP license, DEA license, US work authorization, and Spanish bilingual proficiency. This is a full-time position with competitive pay and benefits.
Qualifications
- MSN/ DNP/ NP certificate required.
- US NP license active and in good standing.
- DEA license required.
- Bilingual Spanish required/supported.
Responsibilities
- Provide exceptional primary care to adults on Medicare.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and HRAs with thorough documentation.
- Coordinate care with other providers, specialists, testing facilities and agencies.
- Lead population health initiatives with the care team.
- Assist with phone triage and outreach.
- Educate patients about health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
- Other duties as assigned.
