Job summary

Oak Street Health, based in Bronx, NY, seeks a Nurse Practitioner (Spanish Required) to deliver effective, value-based primary care to adults on Medicare, helping keep them healthy and out of the hospital.

NPs work with a collaborative care team, perform annual wellness visits, coordinate with specialists, and participate in population health initiatives within outpatient clinics. 40 hours weekly, full-time position.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in one of the listed NP specialties.
• Active, non-probationary NP license in the state.
• Active DEA license.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and HRAs with comprehensive documentation.
• Care coordination with physicians, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions and care plans.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties as assigned.
