Job summary

Healthfirst in Connecticut seeks a Relationship Coordinator to educate referral sources and prospective members about Medicare Advantage products and long-term care services. You will handle inbound/outbound calls, manage enrollment steps in Salesforce, and coordinate appointments with SHP and CompleteCare teams to support eligible individuals.

Qualifications

• English reading and speaking proficiency required.
• Customer service experience preferred.
• Ability to work 8.5 hour shifts across varied hours including enrollment peak periods.
• Reliable internet for remote work.
• Experience handling PHI with security awareness.

Responsibilities

• Handle inbound/outbound calls per scripts and SLAs.
• Create and follow up on LTC leads in Salesforce.
• Schedule appointments for SHP Intake Nurses and HF CompleteCare staff.
• Document interactions and outcomes in Salesforce and enrollment databases.
• Coordinate fulfillment kits and information materials for prospective members.
• Troubleshoot LTC enrollment issues and support remote operations.
