Job summary

Make-A-Wish Texas Gulf Coast and Louisiana is seeking a Medical Outreach Coordinator in Stafford, TX, on a grant-funded contract. The role supports provider engagement, referral readiness, medical outreach activities, and intake coordination to connect medically eligible children to the wish journey.

Under the Medical Outreach Manager, you will liaise with healthcare providers, hospital staff, referral partners, and families while educating providers, following up on referrals, and ensuring

Qualifications

• Associate degree required; bachelor's degree in social services, healthcare administration, child development, nonprofit management, marketing or communications preferred.
• Experience communicating with healthcare professionals and families about sensitive information.
• Experience coordinating events and outreach activities; proficiency with Microsoft Office and Salesforce is preferred.

Responsibilities

• Support implementation of grant-funded medical outreach initiatives and referral readiness activities.
• Build and maintain positive relationships with physicians, nurses, social workers, care coordinators, and other referral partners.
• Participate in hospital outreach rounds, provider meetings, lunch-and-learn sessions, and outreach events.
• Assist with scheduling, coordinating, and supporting provider education activities and engagement opportunities.
• Maintain regular communication with referral sources to reinforce referral processes and eligibility awareness.
• Develop and distribute provider education materials, referral resources, workflow guides, posters, and awareness tools.
• Support expansion of the referral model to additional clinics.
• Track and monitor referrals and coordinate between providers and Make-A-Wish intake teams.
• Assist in identifying referral barriers and elevate concerns to the Bilingual Wish Manager.
• Provide bilingual outreach support to Spanish-speaking families and healthcare partners.
