Job summary

National Youth Advocate Program (NYAP) is seeking a Medical Coordinator for La Jornada to arrange and coordinate medical, dental, mental health, and pharmacy services for children in care. The role involves documenting services, coordinating appointments, and maintaining confidential health records in compliance with policies.

The candidate will work with care teams, support foster parents, and ensure timely intake/discharge processes, including COVID-19 testing and medical documentation

Qualifications

• Licensed health professional with required licensure or degree and good driving record.
• Bilingual fluency in English and Spanish is required.
• Minimum 1 year in public health/healthcare with care coordination or related duties.
• Experience in child welfare or related social services is preferred.

Responsibilities

• Coordinate medical, dental, mental health, and pharmacy services for children in care.
• Maintain confidential health records and ensure PHI compliance.
• Schedule and coordinate medical appointments and tests; prepare documentation packages.
• Collaborate with cross-functional teams to ensure timely care and treatment plans.
• Provide education to foster parents on medical policies and care procedures.
• Complete and submit required documentation within deadlines.
