Job summary

Sunnyside Community Services is seeking a Medicaid Specialist to guide caregivers through Medicaid eligibility and enrollment, ensuring access to benefits and long-term care services. The role involves preparing applications and providing ongoing support, with assessments possible in the SCS office or caregivers’ homes.

Qualifications

• AA/AS degree required; minimum 2 years of relevant experience.
• Knowledge of Medicaid eligibility rules at federal, state, and city levels.
• Bilingual in Spanish, Mandarin, or Cantonese.

Responsibilities

• Assess clients’ eligibility for Medicaid and related benefits.
• Educate caregivers on eligibility and enrollment procedures.
• Prepare, submit, and monitor Medicaid applications with documentation.
• Follow up with caregivers to track status.
• Maintain client records per program and regulatory requirements.
• Build relationships with community organizations and referral sources.
• Participate in community events and presentations.
• Attend interdisciplinary team meetings as needed.
