Job summary

HealthFirst Family Care Center, Inc. is seeking a full-time Licensed Practical Nurse (LPN) in Fall River, MA to join a diverse pediatric and adult patient population. Under nursing leadership, you will perform assessments, provide treatment, and support continuity of care with providers.

The role requires MA LPN licensure, BLS/CPR, EMR experience, and strong communication within a multi-cultural team. Schedule is Mon–Fri 9:30 am–6:00 pm with rotating Saturdays, and a pay range of $31–$37.50 per

Qualifications

• License to practice as an LPN in Massachusetts.
• 2+ years of LPN experience in a Community Health Center is a strong plus.
• Current BLS/CPR certification.
• Current and complete credentialing as required by licensing boards and agencies.
• Bi-lingual/tri-lingual skills (English, Spanish, Portuguese) preferred.
• Experience with an Electronic Medical Record (EMR).
• Familiarity with diverse cultural/socio-economic backgrounds.

Responsibilities

• Conduct patient assessment, treatment, and general care during visits.
• Collaborate with Providers to co-manage patient care for continuity.
• Maintain organized work area and support a positive patient experience.
• Communicate effectively with patients and team while handling confidential information.
