AltaMed Health Services Corp. is seeking a Linkage to Care Coordinator II to facilitate seamless enrollment into medical care and ancillary services for newly diagnosed HIV and Hepatitis C patients, as well as patients transferring care to AltaMed.

The role focuses on reducing barriers to care by linking clients with medications, pharmacy services, and access to Behavioral Health and substance use services.

Qualifications

• Bilingual English/Spanish required.
• Bachelor's degree in healthcare, social work, or a related field.
• Previous experience in healthcare coordination, case management, or social services preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
