Job Overview

The Linkage to Care Coordinator II is responsible for the seamless enrollment into medical care and ancillary services of all newly diagnosed HIV and Hepatitis C patients and patients transferring care to AltaMed.The Coordinator supports patients to reduce barriers to care by facilitating access to medications and linkage to pharmacy services, connecting clients to Behavioral Health and substance abuse services, and linking clients with appropriate case management services and prevention counseling.

Minimum Requirements

• Bilingual English/Spanish required.
• Bachelor's degree in healthcare, social work, or a related field.
• Previous experience in healthcare coordination, case management, or social services preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$26.91 - $33.53 hourly
