Job summary

Jobtailor is seeking a capable Care Coordinator in Miami to coordinate care, educate members, and facilitate access to services. You will use care management tools and analyze data to identify risks, develop and implement care plans, and collaborate across teams to address barriers.

The role requires at least 1 year of behavioral health or long-term care experience, bilingual English/Spanish, and willingness to travel up to 75%. Residency in Miami-Dade County is required.

Qualifications
