Job summary

Community Health Centers of America in Fresno seeks a licensed clinical social worker (LCSW) to join our outpatient team. The LCSW provides integrated behavioral health services, psychosocial assessments, crisis intervention, and short-term therapy in a community health setting.

You will collaborate with primary care providers and care teams to address social determinants of health, develop care plans, document in the EHR, and participate in quality initiatives while serving diverse, underserved

Qualifications

• MSW from CSWE-accredited program.
• Active, unrestricted LCSW license in CA (BBS).
• Minimum of 1 year post-licensure clinical experience preferred.
• DEA and BLS certifications.
• Bilingual English/Spanish preferred or required by clinic demographics.
• Experience with Medi-Cal/Medicare compliance.

Responsibilities

• Provide integrated behavioral health services and brief interventions during medical visits.
• Perform psychosocial assessments and develop short-term, culturally competent treatment plans.
• Deliver evidence-based therapies (CBT, MI, SFBT, trauma-informed care).
• Conduct crisis safety assessments and coordinate emergency psychiatric services when needed.
• Screen for social determinants of health and connect patients to community resources.
• Maintain EHR/EMR documentation per guidelines (e.g., Epic, NextGen).
• Participate in multidisciplinary team huddles, case conferences, and quality improvement.
