Job summary

Community Health Centers of America (CHCA) in Fresno, CA seeks a Licensed Clinical Social Worker (LCSW) to join our Federally Qualified Health Center team. The LCSW will provide integrated behavioral health services in an outpatient primary care setting, perform psychosocial assessments, and contribute to short-term therapy for diverse patients.

You will collaborate with PCPs, care managers, and psychiatrists, participate in care coordination, maintain EHR documentation, and support quality

Qualifications

• Master’s degree in Social Work from CSWE-accredited program.
• Active LCSW license in California (BBS).
• Minimum 1 year post-licensure clinical experience preferred.
• Bilingual English/Spanish helpful based on clinic demographics.
• DEA and BLS certifications current.

Responsibilities

• Provide integrated behavioral health services within outpatient primary care.
• Perform psychosocial assessments and develop short-term treatment plans.
• Deliver evidence-based therapies (CBT, MI, SFBT, trauma-informed care).
• Coordinate crisis management, safety planning, and referrals as needed.
• Collaborate with primary care, care managers, and psychiatrists.
• Maintain compliant EHR documentation and adhere to guidelines.
• Screen for social determinants of health and connect to community resources.
