Job summary

Washington County is seeking a Jail Care Coordinator to support adults in custody with serious mental illness, assess needs in jail, develop care plans, and connect them with community resources to reduce recidivism. The role collaborates closely with forensics, jail staff, attorneys, Medicaid/CCO's, and medical providers, housing partners, and other agencies.

After release, brief follow-up ensures service connections.

Qualifications

• Master's degree in Counseling, Psychology, Social Work, or related field.
• Three years of case management or services coordination experience.
• Must be QMHP-certified or hold an approved clinical license per Oregon rules.
• Successful completion of background checks (post offer).
• Bilingual Spanish skills are encouraged for eligibility for differential pay.

Responsibilities

• Meet clients in jail to assess needs and develop care plans.
• Connect individuals with community resources to divert further legal involvement.
• Collaborate with forensics, jail staff, attorneys, Medicaid/CCO's, and providers.
• Provide post-release support to ensure service connections and reduce recidivism.
• Offer consultation/education to corrections partners regarding behavioral health.
