Qualifications

• Master’s degree required with 3+ years in case management or services coordination.
• Must be certified by the Mental Health and Addictions Certification Board of Oregon or hold an approved clinical license.

Responsibilities

• Coordinate care for adults with severe and persistent mental illness.
• Connect clients to services and supports to stabilize symptoms and navigate the legal system.
• Coordinate referrals for residential and outpatient treatment and assess system needs.
• Attend and facilitate team meetings for transitions in levels of care and placements.
