Job summary

Redwood Community Services, Inc. is seeking an Integrated Care Manager - Spanish Speaking in Lakeport, CA.

This is a full-time, non-exempt role coordinating health and related services for participants with complex needs, guiding them through a comprehensive care plan. You will engage clients in group and individual settings, connect them to medical, behavioral, dental, and social supports, document care, and collaborate with providers.

Qualifications

• Care Manager I: <2 years direct social work experience.
• Care Manager II: 2–4 years in a mental health direct services.
• Care Manager III: ≥4 years direct client services; up to 2 years education substitution.
• Must be fluent in Spanish.
• Basic knowledge of Microsoft Office (Excel/PowerPoint/Outlook).
• Willingness to work a flexible schedule, including weekends and evenings.

Responsibilities

• Outreach to connect potential participants to Enhanced Care Management (ECM).
• Assess needs and draft individualized care plans with participants.
• Work with clients in group and individual settings to address barriers to the plan of care.
• Refer clients to medical and mental health care and assist with scheduling.
• Advocate for clients and coordinate with providers to maintain engagement in care.
• Take and record vital signs and conduct wellness screenings.
• Document services provided in a timely manner.
• Collaborate with partner providers on treatment progress and needs.
• Develop knowledge of local resources and update as needed.
