Job summary

La Clinica de Familia Inc in southern New Mexico is seeking a co-located, integrated care behavioral health provider to support the primary care team in managing the health of enrolled patients.

Deliver brief, consultation-based services within an integrated care model in the primary care clinic, collaborating with PCPs and a multi-disciplinary team to improve psychosocial health outcomes.

Qualifications

• Master's Degree in Social Work or related field and NM licensure required.
• Bilingual English/Spanish and strong communication abilities.
• Experience with brief, crisis and evidence-based interventions in primary care.

Responsibilities

• Provide consultation services to patients referred by the PCP team.
• Support care management and health outcomes within an integrated care model.
• Collaborate with the multidisciplinary team and PCP owner of care.
